Position: Home Health Clinical Manager (RN)

Responsibilities
  • Oversee day-to-day office and clinical operations, ensuring continuity of services, completion of daily and weekly tasks, and appropriate staff coverage during absences.
  • Supervise clinical and administrative staff, including RN and PT Case Managers; monitor action screens, schedules, productivity, and overall workflow.
  • Develop and manage on-call schedules, review on-call logs, provide administrative on-call support, and ensure smooth off-hours operations.
  • Review, approve, and follow up on clinical documentation including plans of care, evaluations, OASIS, 485s, chart audits, complaints, and quality improvement reports; address documentation deficiencies as needed.
  • Ensure individualized, appropriate plans of care are executed as written and reassessed per regulatory and physician requirements; ensure proper documentation for transfers and discharges.
  • Manage intake, referral, non-admission, internal transfer, admission, and billing processes, including non-Medicare referrals, to ensure timely reimbursement and minimize outstanding claims.
  • Approve schedules, payroll transmittals, non-visit activity, and accounts payable in accordance with company protocols and timelines.
  • Complete and distribute required audits and reports; address internal audit findings and maintain required clinical, operational, and HR performance benchmarks.
  • Ensure compliance with state, federal, and internal regulations; maintain survey readiness and support all external audits.
  • Lead and participate in staff meetings, aide in-services, case conferences, and ongoing education initiatives.
  • Support field operations as needed, including patient visits and participation in on-call rotation when applicable.
  • Participate in community outreach, disaster preparedness planning, drills, and emergency response; serve as Disaster Coordinator in the absence of the Administrator.
  • Remain available during operational hours and perform additional duties as assigned by the Branch Director.
Qualifications
  • Graduate of an accredited school of nursing. Bachelor's degree preferred, with a minimum of 3+ years of home health care experience.
  • Must have a minimum of 2+ years of nursing experience within the last 36 months.
  • Medicare home health experience required.
  • Current licensure in State, CPR certification
  • Must have a good understanding of the Federal, State and local laws/regulatory guidelines governing the operation of a Home Health Mission Healthcare.
  • Have valid driver’s license with no restrictions and auto insurance meeting state’s insurance coverage minimums.
  • Responsible for supervising home health aides. Supervisory experience not required but preferred.
  • 2 years of Home Health experience required. 3+ years of Home Health experience preferred.
  • Preceptor experience preferred.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
