What You Need to KnowIf you meet these qualifications, we want to meet you!
- Graduated from an approved school of professional nursing
- One year experience as a licensed practical nurse / Palliative experience preferred, required in California
- Licensed to practice as a Licensed Practical Nurse in the state of agency operation
- Possess and maintain valid CPR certification while employed in a clinical role, preferred
- Must be a licensed driver who can travel to all business locations
- Meet the regulations and requirements of the state(s) in which program provides services
- Under the direction of an NP or physician, assist in execution of the care plan, including routine and emergent visits
- Contributes to the ongoing care planning process, especially regarding medical needs, with the care team and other agencies as required
- Initiates and maintains communication with the NP, interdisciplinary team, Palliative Director, and contracted medical groups to coordinate optimal care and use of resources for the patient/family
- Provides support for the Palliative patient and family as new clinical needs are identified or questions arise, with referral to the clinician as appropriate
- Gathers information regarding the patient for appropriateness for other service lines that will best meet patient needs such as Hospice
