We’re currently seeking a dedicated Social Worker BSW to join our Case Management Department at our Ontario location
.

Assess patients and families and provides intervention and treatment to support patients. Address psychosocial needs and patterns of adjustment throughout the hospitalization and collaborates with the multidisciplinary team to formulate treatment plans to maximize emotional, social and physical well-being. Provides information and education regarding health care and community resources.

• Current State of Ohio Social Work license.
