Job Summary
Infusion Nurse I – licensed registered nurse providing basic direct patient care to patients in the home or alternate infusion site, ensuring patient safety, continuity, and compliance under a physician’s plan of care. Operates under close direct supervision of the designated supervisor.
Responsibilities- Initiate, develop, and implement basic nursing plan of care treatments, evaluating patient progress toward goals; provide direct patient care, administer medications, and educate patients and families/caregivers.
- Assess patient needs and physical status at each visit, re-evaluate status through reassessment and supplemental physician orders, and communicate significant changes to the physician promptly.
- Teach patients and families using a patient‑centered approach to achieve independence with prescribed therapy.
- Effectively communicate with Naven Health’s Clinical team, medical providers, patients, and families to facilitate continuity of care; prepare real‑time documentation during visits and submit to nursing department for billing compliance.
- Coordinate discharge planning, prepare discharge summaries, and report patient needs, progress, and goals when transferring care.
- Demonstrate compliance with agency operations, policies, professional standards, and regulatory guidelines.
- Maximize work efficiency through computers and automation technologies to validate orders and manage visit schedules.
- Respond promptly to patient requests and initiate emergency procedures as needed.
- Accept accountability for practice through ethical and professional conduct; follow established programs and procedures aligned with Naven Health’s mission.
- Safeguard patient and proprietary information, adhering to HIPAA regulations.
- Speak knowledgeably about Naven Health’s services and instruct patients about related plan of care.
- Complete required training, in‑services, and continuing education to maintain competency and licensure; provide oversight/coordination of paraprofessionals and may act as Supervisor when applicable.
- Participate in multidisciplinary team conferences and provide precepting, training, and mentoring to other nurses.
- Participate in nursing on‑call responsibilities as applicable.
- Perform other related duties as directed by supervisor.
- Active and unrestricted RN license in the state of practice.
- Minimum of 0–2 years of infusion nursing experience.
- Current CPR certification.
- Competency in patient care standards and infusion skill sets applicable to agency programs and service needs; advanced certification and training as applicable.
- Valid driver’s license in the state of practice and access to a vehicle with proof of liability insurance.
- Basic knowledge of computer operating systems and software applications; ability to apply technology tools for effective communication and documentation.
- Ability to lift up to 50 lbs (max 75 lbs); may exert up to 20 lbs occasionally or 10 lbs frequently; tasks include walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching, grasping, pushing, pulling, lifting, and carrying; fine motor skills and visual acuity required.
- Potential occupational exposure to blood, body fluids, and other infectious materials; possible exposure to hazardous substances and unpredictable home environments.
- Safety requirements include closed‑toe shoes, facemask, goggles, gown, and/or gloves; functional communication device for routine and emergency situations.
Willing to travel 100% of the time for business purposes.
Preferred Qualifications- Bachelor of Science in Nursing preferred.
- 1 year of previous experience as an RN in a home health or acute care setting.
Salary to be determined by applicant’s education, experience, knowledge, skills, and abilities, as well as internal equity and market data.
Pay range: $36.89 – $61.49.
