No bedside nursing. 16 guaranteed hours per week. Wednesdays and Thursdays. $35 to $40/hour plus mileage reimbursement.

Assisting Hands Home Care, South Jacksonville is seeking a compassionate, organized RN to conduct client assessments, develop individualized care plans, and complete documentation. Many of the clients served in this role receive home care services through the Department of Veterans Affairs (VA).You'll evaluate client needs, coordinate non-skilled home care services, and equip caregivers to provide safe, consistent, compassionate support. You'll work independently, with the backing of an experienced office and nursing team.

Mileage reimbursement: $0.70 per eligible business mile, in accordance with agency policy.

What You'll DoConduct initial in-home assessments covering daily living needs, functional limitations, safety concerns, and personal care requirements.

Develop individualized care plans with clear, practical instructions for caregivers.

Complete periodic reassessments and update care plans as client needs change.

Maintain accurate, timely, and confidential electronic documentation according to agency procedures and applicable VA program requirements.

Communicate with clients, families, caregivers, and office staff to support continuity of care.

Identify and report significant changes in client condition, safety concerns, or unmet care needs.

Guide caregivers on client-specific care plans and service expectations.

Coordinate visit scheduling and follow-up needs with the office team.

Qualifications

Required:

Current, active RN license authorizing practice in Florida.

Minimum of 5 years of experience as a Registered Nurse.

Strong assessment, documentation, organizational, and communication skills.

Ability to work independently and manage a schedule of client visits.

Comfort with electronic documentation systems.

Valid driver's license, reliable transportation, and current auto insurance.

Ability to travel to client homes, primarily in Clay and Duval Counties.

Availability to work Wednesdays and Thursdays.

Preferred:

Experience in home care, home health, geriatric nursing, or case management.

Experience completing client assessments and developing individualized care plans.

Familiarity with VA-funded home care programs and documentation requirements.
