Summary
Educates members on health issues, including nutrition, diet and exercise, and provides linkage to health‑related community resources such as smoking cessation programs. The Health Educator combines wellness knowledge with community service navigation to facilitate appropriate healthcare referrals.
Essential Duties and Responsibilities- Provides linkage services for members with physical, dental, or mental health care needs.
- Under direction and supervision of the Program Director, delivers health education and community linkage.
- Establishes and maintains working relationships with community resources to assist in linkage.
- Develops and teaches wellness‑related classes and groups.
- Serves as a resource to members, community resources, and staff.
- Maintains HIPAA compliance and ethical standards, including professional boundaries with all staff and clients.
- Performs other duties as assigned to support the program and organization.
- Provide exceptional customer care.
- Identify and build upon client and community strengths.
- Support clients' steps toward recovery and wellness.
- Create an inclusive organizational culture that respects client diversity.
- Value ongoing learning to better serve clients and lead in the industry.
- Research and apply industry best practices to ensure optimal outcomes.
Works under immediate supervision; no supervisory responsibilities.
Education and Licensing Requirements- Graduate from an accredited school for health professionals; bachelor’s degree preferred, or certification without a bachelor’s such as LVN, MA, or another associated health licensed professional.
- Valid California driver’s license and auto insurance; proof of education required.
- Preferred:
One year acute care and community clinic experience. - Experience working with severely and persistently mentally ill population.
- Intermediate level PC skills.
- Excellent organizational, interpersonal, computer, and communication skills.
- Ability to prioritize and manage time effectively.
- Office environment; extensive sitting with periodic standing and walking.
- May need to lift/move up to 20 pounds.
- Significant use of computer, tablet, phone, and general office equipment.
- Requires adequate visual acuity and fine motor skills.
- Effective verbal and written communication skills.
- May require 20% off‑site travel; not a remote position.
- Uses personal vehicle as required.
