Your Role on the Team

Position Summary:

The Supervisor of Case Management is responsible for assisting with the alignment of case management competencies and metrics for the assigned area. The CM supervisor provides day‑to‑day frontline supervision of assigned case managers, ensuring high‑level assessment and reassessment to assure that each patient has a plan of care that is focused on high quality, individualized, safe and cost‑effective transitions of care.

The supervisor ensures that the documentation from each case manager in the medical record is complete and reflects the patient’s true severity of illness.
Additional responsibilities include daily assignment, assistance with difficult discharges, scheduling and orientation of new staff. The supervisor serves as a role model for excellent service by training and reinforcing behaviors and standards among staff that lead to positive interactions with patients and timely resolution of patient complaints. The supervisor promotes collegiality and teamwork by serving as a liaison between assigned units and other departments or disciplines across the Medical Center with a focus on patient throughput and decreasing length of stay.

Required Qualifications:

  • Must possess the skill, knowledge, and ability essential to the successful performance of assigned duties
  • Must possess complete understanding of acuity levels for specific patient population
  • Must demonstrate customer service skills appropriate to the job
  • Must be willing to work a flexible schedule, including weekends, and on call after hours as needed
  • Minimum 3 years of Case Management experience
  • Familiarity of Joint Commission, CMS, CDPH and CCS requirements
  • Excellent written and verbal communication skills in English
  • Demonstrated leadership, interpersonal skills, and ability to communicate with varied levels of customer audiences
  • Demonstrated leadership skills/experience
  • Demonstrated experience and ability to build effective working relationships and to represent the department effectively in order to accomplish goals
  • Current California RN license
  • Computer literacy and proficiency
  • Bachelor’s degree in relevant field or the equivalent combination of education and experience
  • All external candidates must have a Bachelor of Science in Nursing
  • Ability to manage multiple concurrent projects and maintain a work pace appropriate to the workload
  • Ability to facilitate and lead interdisciplinary rounds
  • Ability to establish and maintain effective working relationships across the Health System

Preferred Qualifications:

  • Knowledge of University policies and procedures
  • Knowledge of California Children’s Services benefits authorization process (CCS)
  • CCM, ACM or Case Management credentialing
  • Bilingual skills
