Damascus, Sandy, Boring and Welches. Manage and provide skilled occupational therapy to patients and clients with a wide variety of diagnoses and disabilities in home settings, following physician‑directed plans of treatment. • Bachelor’s Degree from an accredited Occupational Therapy Program
• Master’s Degree from an accredited Occupational Therapy Program
• Ph.D. from an accredited Occupational Therapy Program
• Within 30 days of hire:
  National Provider BLS – American Heart Association
• Upon hire:
  Oregon Occupational Therapist License
• Washington Occupational Therapist License (Vendor Managed) – if required upon hire
