Pay and Benefits

Annual Pay Range: $73,699.88 – $93,581.75

Hourly Pay Range: $35.43 – $44.99

Bilingual Spanish/English skills are required and an additional 5 % of base compensation is added to pay.

Qualifications & Experience

Required

Minimum Qualifications:

  • Qualified Mental Health Associate (QMHA) or R as established by the Mental Health and Addiction Counseling Board of Oregon at the time of hire.
  • Minimum of three (3) years of related experience in behavioral health, integrated care, complex case management, or crisis services.
  • Fluency in Spanish and English (oral and written).
  • Basic Life Support (BLS) certificate.
  • Experience working with vulnerable clients and collaborating with community partners, law enforcement, and co‑workers.

Preferred Special

Qualifications:

  • Certification as a Certified Alcohol and Drug Counselor (CADC) I, II, or III.
  • Knowledge of mental health, substance abuse, legal, medical, social, and/or housing resources in the community.
  • Experience in a primary care or behavioral health setting.
  • Proficiency with electronic health record systems (Epic preferred).
Responsibilities
  • Build rapport with clients and serve as the point of contact within the integrated care team.
  • Provide client outreach and support treatment engagement, including encounter‑based outreach.
  • Assist clients and providers with referrals to higher levels of care and serve as the care team liaison with primary care.
  • Facilitate communication between peers, case managers, and clinical staff.
  • Lead daily interdisciplinary huddles and manage in‑basket messaging within the electronic health record system (Epic).
  • Support scheduling of client appointments and care team tasks; audit documentation and service plans.
  • Generate, monitor, and complete referrals to internal and external services, collaborating with case managers on linkage to outside providers and community support.
  • Coordinate service coordination for hospital step‑downs, emergency department discharges, and intake scheduling.
  • Participate in program compliance reviews and audit preparations.
Work Schedule

This position works 40 hours during a standard work week of Monday through Friday or an alternate workweek Monday through Thursday or Tuesday through Friday. The exact schedule will be discussed with the selected candidate.

Hybrid Telework

Eligibility for hybrid on‑site/telework is subject to the department’s business needs. The exact amount of on‑site/telework and schedule will be discussed with the hiring manager.
