Case Manager Salary Range
Annual Pay Range: $61,374.91 - $77,335.94
Hourly Pay Range: $29.507168 - $37.180740
NOTE:
Bilingual Spanish/English skills are required; an additional 5% of base compensation is added to pay.
Health Centers-Behavioral Health provides licensure supervision for regular status employees pursuing licensure; group supervision is available for CADC hours. Clackamas County Health Centers are Federally Qualified Health Centers. Loan forgiveness options may be available through NHSC or PSLF.

JOB DETAILS AND QUALIFICATIONSClackamas County Health Centers seeks a Bilingual (Spanish) Case Manager for the Health Center Behavioral Health Clinic in Milwaukie, Oregon. This role supports individuals with mental health and/or substance use challenges through direct care and service coordination, within a multidisciplinary team.
The Case Manager provides comprehensive care coordination and case management, including skills training, medication education and support, resource navigation, and connection to treatment and recovery services. Services are delivered across clinics, homes, hospitals, care facilities, and community locations. Must engage a diverse client base professionally in high-need environments.

Required Minimum Qualifications/Transferable Skills
- Qualified Mental Health Associate (QMHA) or QMHA-R as established by MHACBO (per Oregon Administrative Rule at time of hire)
- Fluency in Spanish/English, oral and written
- Must possess and maintain a Basic Life Support (BLS) certificate
- Experience providing case management to clients with severe and persistent mental illness (SPMI) and/or substance use disorders (SUD)
Preferred Special Qualifications/Transferable Skills
- Experience in primary care or behavioral health care setting
- Experience serving vulnerable clients
- Experience with Evidence Based Treatment Practices
Pre-Employment Requirements

- Criminal history check (may include fingerprinting)
- Must have valid driver’s license; driving may be required for County business

TYPICAL TASKSDuties may include but are not limited to the following:
- Coordinate care for clients with mental health and/or addiction issues, including referrals, monitoring treatment, and discharge planning
- Collaborate with doctors, nurses, and insurance companies to maintain client stability and transitions between services
- Interview clients, families, and staff to inform treatment plans and determine services
- Maintain accurate case records and documentation per agency policies
- Provide counseling and support on daily living skills, job readiness, and medication management
- Collaborate with healthcare providers and social services to secure necessary services
- Advocate for clients by coordinating with families, providers, and agencies; respond to emergencies
40 hours per week, standard Monday–Friday schedule. Specific hours discussed at offer stage. Schedule options include Four 10-hour shifts. Hybrid on-site/telework may be available subject to department needs; telework location restrictions apply (Oregon or Washington; Washington residents must be on-site 50% of time).
