Licensed Practical Nurse (LPN) – Avamere Rehabilitation of Oregon City

Status: PRN – am and pm shifts available

Location: Avamere Rehab of Oregon City – 1400 Division St, Oregon City, OR 97045

Responsibilities
  • Complete required record keeping regarding admitting, transferring, and discharging residents.
  • Coordinate with nursing staff and support personnel daily to plan shifts’ services, programs and activities to maintain residents’ care regimens.
  • Prepare and administer medications and review medication cards for completeness and accuracy.
  • Plan, organize, develop, implement and interpret programs, goals, objectives, policies and procedures necessary for providing quality care.
  • Provide leadership to nursing personnel and conduct daily rounds of your unit to ensure staff provides direct care in accordance with residents’ care plans and wishes.
  • Participate in facility surveys by authorized government agencies.
Qualifications
  • Must possess a nursing degree from an accredited college or university.
  • Must hold a current, unencumbered, active LPN license in this state.
  • Knowledgeable of nursing and medical practices, laws, regulations, and guidelines that pertain to nursing care facilities.
  • Must chart nursing notes informatively and descriptively, reflecting the care provided and residents’ response to care.
  • Experience with Electronic Medical Records and computer documentation systems.
  • Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
  • Must speak, read, and write English fluently.
  • Must have an active CPR/BLS certification.
