American Traveler is seeking an LPN for a skilled Home Health assignment requiring an active Oregon license and at least 1 year of skilled Home Health experience (license required if applicable).
Requirements- Active Oregon license required before start date; temporary licenses not accepted (license required if applicable)
- BLS certification required
- Minimum of 1 year of skilled Home Health experience required
- HCHB (Homecare Homebase) proficiency required
- OASIS experience required for staff completing Start of Care (SOC)
- Reliable personal transportation and active driver's license required
- First-time travelers not accepted
- Skilled Home Health setting with a 1:1 patient ratio
- Approximately 30 points per week, up to 6 patients per day
- 8-hour day shift schedule (8:00 AM-4:30 PM), 5 days per week
- 4x10 and 3x12 schedules are not available
- Weekend support may be required
- Uses Home Care Home Base (HCHB), Remote Client for Home Health on laptop, and Rover phone application
- Facility makes efforts to group patient visits and minimize driving, though service area may extend up to approximately 60 miles or 60-90 minutes of total driving per day
- On-call rotation may be required; details discussed during interview
- Business casual attire required; no jeans; scrubs permitted but not required
- Free parking available; mileage reimbursed at current IRS standard rate
- Up to 3 days of RTO may be automatically approved; blackout periods include weeks of Thanksgiving, Christmas, and New Year's
- RTO requests must be submitted at time of application
- Travel pairs accepted, but each traveler assigned to own territory and must have own transportation
- Former permanent employees must be separated for a minimum of 6 months and approved by HR before returning as a traveler
- Local candidates accepted within a 50-75 mile radius at the same rate
- Monday orientation required, including completion of billable modules
