Transition Specialist

• Support care coordination teams and providers in developing effective transition plans for members
• Assist with the transition for members based on enrollment or care changes
• Work with care management teams to identify new members requiring transition services
• Educate members and families on services, requirements, and limitations related to transition planning
• Track and maintain transition metrics to identify trends and ensure proper documentation

• Bachelor's degree required
• 2 – 4 years of related experience in healthcare or a similar field
• Strong understanding of healthcare services and transition processes
• Excellent communication and interpersonal skills
• Ability to work independently and manage multiple tasks effectively

• Previous experience in care coordination or case management
• Familiarity with Medicaid and Medicare services
• Experience developing educational materials for healthcare services
• Knowledge of community resources for healthcare transitions
• Skills in data tracking and analysis for process improvements
