PT - Physical Therapist

Salem, OR, and surrounding areas (South Salem, West Salem, Corvallis, Albany, Sweet Home, Lebanon)

This is a 13-week contract position.

Day shift, 8-hour shifts (08:00 - 16:30). Note that this facility cannot accommodate 4x10s or 3x12s.

The Home Health Physical Therapist (PT) will be responsible for providing therapeutic services to patients in their homes, ensuring adherence to the highest standards of care.

• Conduct patient assessments and develop individualized treatment plans.
• Administer appropriate therapeutic interventions to enhance patient mobility and function.
• Maintain detailed and accurate documentation using EPIC Remote Client and Oasis.
• Collaborate with interdisciplinary teams to provide comprehensive patient care.
• Manage a caseload of up to 6 patients per day while meeting productivity standards.

• Minimum of 1 year of skilled home health experience.
• Valid state license as a Physical Therapist.
• BCLS certification required.
• Experience with Oasis charting is preferred.
