Advanced Practice Clinician (NP/PA) - Senior Community Care

Optum | Lane County, OR | Full-Time | Field-Based

ooking for a role that offers both meaningful patient care and schedule flexibility? Optum's Senior Community Care team is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal care to medically complex patients in skilled nursing facilities throughout Lane County.

As the primary Advanced Practice Clinician for your patient panel, you'll have the flexibility to manage your own daily schedule while delivering high-quality, value-based care. With no productivity quotas, RVU requirements, call responsibilities, weekends, or holidays, you can focus on your patients while maintaining a healthy work-life balance.

  • Competitive base salary plus performance bonus incentives
  • $2,500 Relocation Assistance
  • No call, weekends, or holidays
  • 23 days of PTO plus 9 paid holidays
  • No RVU requirements or productivity quotas
  • Manage your own patient panel and daily schedule
  • Flexibility to structure your day across assigned facilities

  • Maintain accurate and timely clinical documentation
Work-Life Balance Without Compromising Patient Care

Optum's value-based care model is designed to support clinicians, not volume metrics. Providers have the flexibility to manage their schedules, prioritize patient needs, and focus on delivering thoughtful, longitudinal care rather than meeting productivity targets.

This role is ideal for clinicians who value independence, flexibility, and the ability to build meaningful patient relationships while maintaining predictable work hours and time outside of work.
