Working Title: Mental Health Occupational Therapist
Program: Community Support Services – EASA
Location: On-site, The Dalles (Serving Wasco, Hood River, and Sherman Counties)
Hours/ FTE: Flexible, total 7.5 hours/week (0.2 FTE)
Pay Range: Dependent on experience
Reports To: Program Manager
FLSA Status: Exempt
Responsibilities- Provide professional evaluation, treatment, referral, and monitoring services within the scope of Occupational Therapy.
- Deliver OT services for the EASA program, serving youth and young adults ages 15-25 who are experiencing their first episode of psychosis; aim to reduce symptoms and improve functioning.
- Collaborate with a multidisciplinary team; conduct assessments; develop OT treatment plans and interventions addressing sensory-motor processing, cognitive, and environmental factors to support mental health, education, and vocation services.
- Perform client evaluations, formulate diagnoses, and communicate results and recommendations; complete documentation and releases as needed.
- Create and implement Occupational Profiles, Performance Analyses, Evaluations, and Treatment Plans; coordinate OT with other therapies; adapt environments to support clients’ work, school, and independent living skills.
- Participate in rounds, administrative meetings, and case staffing; provide consultation or follow-up to clients, families, employers/schools, and multidisciplinary teams; may train caregivers and co-facilitate psycho-education groups.
- Ensure safety, effectiveness, and quality of OT services; complete required documentation (progress notes, treatment plans, state forms, outcome measures); maintain credentialing requirements.
- Prepare timely clinical documentation in the agency system (treatment records, service plans, progress notes) and maintain integrity of the electronic records.
- Maintain regular and reliable attendance; participate in all team and agency meetings; provide transportation for clients using agency vehicles as required.
- Must meet QMHP credentialing requirements as per OARs (8).
- Master’s Degree in Occupational Therapy.
- Two years of professional experience in a related field, including at least 1 year as an Occupational Therapist; prior experience with adolescents and/or young adults up to 25.
- Licensed as an Occupational Therapist by the State of Oregon; criminal background check required.
- Bilingual Spanish/English preferred.
- Must pass criminal history checks as required by ORS and OAR to .
- Valid Oregon or Washington driver's license, acceptable driving record, and ability to travel between business offices, meetings and trainings.
