Build Partnerships
- Strengthen Communities
- Expand Access to Care
What if the right partnership could help a family access behavioral healthcare, connect someone experiencing homelessness with essential services, or bring medical care closer to an underserved community?
At Northwest Human Services (NWHS), we believe healthier communities start with collaboration, compassion, and access to care. As a nonprofit community health and social services organization serving Marion and Polk counties, we support individuals and families navigating complex health and social needs.
We are seeking a Community Partnerships Manager to build strategic relationships, bring organizations together, and turn shared goals into meaningful community impact. You will collaborate with healthcare providers, public agencies, schools, community organizations, funders, and local leaders to strengthen service connections, address barriers, and expand access to care.
Develop Partnership Strategies
- Create and maintain an annual partnership plan aligned with NWHS priorities and community needs.
- Identify service gaps, referral barriers, and opportunities for collaboration.
- Advise leadership on emerging needs, partnership opportunities, and external risks.
Connect Partners and Coordinate Solutions
- Facilitate community forums, listening sessions, and cross-sector meetings.
- Build collaboration among healthcare, housing, behavioral health, education, public agencies, and culturally specific organizations.
- Develop partnership agreements, referral pathways, and coordinated service approaches with clear goals, responsibilities, and outcomes.
- Address implementation barriers and ensure partners follow through on commitments.
Advance Community-Informed Care
- Identify barriers affecting underserved populations, including people experiencing homelessness, rural communities, and individuals with complex health and social needs.
- Use community feedback, needs assessments, and performance data to recommend service improvements.
- Support innovative care models, including mobile, rural, shelter-based, school-linked, and co-located services.
Strengthen Community Engagement and Advocacy
- Represent NWHS at community events, coalitions, resource fairs, and stakeholder meetings.
- Partner with Communications to increase awareness of NWHS services and community impact.
- Support relationships with government officials and monitor policy and funding developments affecting access to care.
- Identify collaborative grants, contracts, and sponsorship opportunities with program leaders and the grants team.
Measure Impact and Ensure Accountability
- Establish partnership goals, work plans, and key performance indicators (KPIs).
- Track progress, partner commitments, outcomes, and risks.
- Prepare reports and present recommendations to leadership.
