American Traveler is hiring an LPN for a skilled Home Health position requiring 1+ year of Home Health experience and active Oregon license (license required if applicable).
Requirements- Active Oregon LPN license required before start date; temporary licenses not accepted (license required if applicable)
- Minimum 1 year of skilled Home Health experience required
- BLS certification required
- OASIS experience required for staff completing Start of Care (SOC)
- Proficiency in HCHB (Homecare Homebase) required
- Reliable personal transportation and valid driver's license required
- Skills required include medication administration, ear/eye drops, IM & SQ injections, NG/G tube care, oral and subcutaneous medication administration, inhaled medications, skin care, and wound care/wound vac
- Skilled Home Health setting with a 1:1 patient ratio
- Caseload of approximately 30 points per week, up to 6 patients per day
- Day shift schedule, 5x8 hours (8:00 AM-4:30 PM)
- 4x10 and 3x12 schedules are not available
- Weekend support may be required
- On-call rotation may be required, to be discussed during interview
- Uses Home Care Home Base (HCHB) for documentation, Remote Client for Home Health on laptop, and Rover phone application
- Responsibilities include resumption of care and routine home health visits
- Facility groups patient visits to minimize driving; service area may extend up to approximately 60 miles or 60-90 minutes of driving per day
- Mileage reimbursed at current IRS standard rate
- Business casual attire required; no jeans; scrubs permitted but not required with no specific color
- Free parking available
- First-time travelers accepted
- Local clinicians accepted within a 50-75 mile radius at the same rate
- Up to 3 days of RTO may be automatically approved
- Blackout periods apply during the weeks of Thanksgiving, Christmas, and New Year's
- Former permanent employees must be separated for a minimum of 6 months and approved by HR before returning as a traveler
- Travel pairs accepted, with each traveler assigned to their own territory and required to have their own transportation
