Nurse Practitioner, Palliative Care in Home (PRN/OCC, Variable Hours and Shifts)

OverviewProvides primary care to patients under the supervision of a physician preceptor, following established medical protocols. Providers must be comfortable performing visits in different settings such as patient homes, Assisted Living Facilities, Nursing Facilities, etc.

- Maintains established departmental policies and procedures, objectives, quality assurance program, and safety standards.
- Provides direct patient care, evaluates outcomes, consults with other specialists as required, and adjusts processes as indicated to ensure optimal patient care.
- Collaborates with physician preceptor to formulate and review medical protocols.
- Obtains complete medical history of the patient.
- Identifies health hazards in the patient’s environment as well as patient risk factors; considers cultural patterns, economic status, and patient resources in plan of care.
- Performs appropriate physical exam as indicated.
- Obtains appropriate laboratory specimens.
- Analyzes objective and subjective data for appropriate diagnosis and treatment.
- Assures treatment and/or plan of care is consistent with established protocols.
- Monitors and ensures accuracy of recording on patient medical records and maintains all required reports, records, statistics, etc. Documents all pertinent information accurately, legibly, and in a timely manner.
- Refers patient to appropriate services which exceed scope of practice of the practitioner.
- Provides health promotion counseling and develops patient teaching information.
- Serves as a resource person for other staff personnel. May supervise clinical staff.
- Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in the field of expertise.
- Attends meetings as required and participates on committees as directed.
Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested. - Performs other duties as assigned.
Minimum Education: None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.
Minimum Experience: None.
Licensure Requirements:
- Current DEA license.
- Must have and maintain certification by the American Nurse Association (ANA) or American Academy of Nurse Practitioners (AANP) with specialty applicable to the area of responsibility – Family Nurse Practitioner, Adult‑Gerontology, Psychiatric‑Mental Health.
- Must meet state licensure requirements.
- CPR required.
- Employee must have a valid Tennessee driver’s license and state mandated minimum insurance coverage.
- Driving record must meet Covenant Health minimum standards at the date of hire and throughout employment tenure.
