Case Manager​/Social Worker

Join our community health clinic as a Case Manager / Social Worker supporting diverse patient populations, including individuals managing chronic health conditions and ongoing care needs.

Location: Orlando, FL (on-site)

Schedule: Monday–Friday | 8:00am–5:00pm

Compensation: $22/hour (direct hire)

• Manage a caseload of approximately 50 patients
• Support patients with care plan navigation, appointment scheduling, and follow-ups
• Coordinate referrals with specialists and third-party providers
• Connect patients to community resources (housing, transportation, insurance, benefits)
• Advocate for patients to reduce barriers to care
• Maintain accurate documentation and case notes in the EHR
• Collaborate closely with providers and clinic staff

• Degree in Social Work, Human Services, Psychology, Public Health, or related field OR equivalent healthcare case management experience
• Prior experience in healthcare, clinic, or community health settings preferred
• Strong communication, organization, and patient advocacy skills
• Comfortable managing a high-volume caseload
• Spanish or Creole bilingual strongly preferred
