Through our Telehealth and RPM services, we ensure that patients have real-time access to healthcare, empowering them to manage their health proactively and avoid unnecessary hospital visits. Our Chronic Care Management programs create tailored care plans for patients with chronic conditions, ensuring ongoing support between office visits. Additionally, our Community Health Initiatives focus on addressing health disparities, making care more accessible to underserved populations.

Field-Based (Orlando and/or Miami)

As a POD Health Community Outreach Representative, you will be responsible for driving growth and expansion of POD Health’s services through provider-based and community referral development. This role focuses on building strong relationships with medical offices, senior-focused organizations, and community partners to generate Start of Care (SOC) opportunities for POD Health. You will serve as a key ambassador for POD Health — educating partners on our services, identifying eligible patients, and ensuring a smooth referral-to-enrollment process in collaboration with internal teams.

• Build and maintain relationships with, but not limited to:
  Primary care practices, Specialist offices, Community health organizations, Senior centers, and advocacy groups.
• Educate referral partners on POD Health services, eligibility requirements, and patient benefits.
• Drive POD Start of Care (SOC) volume by consistently generating and following up on qualified referrals.
  Plan, coordinate, and host community and provider-focused events, including:
  Health education workshops,
• Enrollment events, Senior center presentations, Provider lunch-and-learns
• Represent POD Health at community events, health fairs, provider meetings, and networking opportunities.
• Collaborate closely with intake, clinical, and operations teams to ensure timely follow-up and successful SOC conversions.
• Track and document all outreach activity, referrals, and follow-ups accurately in Salesforce within required timelines.
• Support POD Health growth initiatives, pilot programs, and special outreach campaigns as assigned.
• Maintain compliance with Medicare marketing guidelines and organizational standards.

• Fluency in English and at least one Asian language is required
• Valid driver’s license and reliable transportation for local travel
• Community-based business development, Medicare or value-based care preferred
• Strong understanding of Medicare populations and community healthcare dynamics
• Proven ability to build provider relationships and convert referrals into enrolled patients
• Excellent interpersonal, communication, and presentation skills
• Cultural competency and experience working with diverse communities
• Proficiency in Microsoft Office and Salesforce CRM (or comparable systems)
