LeeKO Home Health LLC is looking for a passionate and dedicated PRN Physical Therapist to join our growing team in Orlando, FL. Responsibilities:- Conduct comprehensive physical therapy evaluations and assessments in patients' homes
- Develop and implement individualized treatment plans to restore function and mobility
- Provide therapeutic interventions including therapeutic exercise, manual therapy, and neuromuscular re-education
- Monitor and document patient progress, adjusting care plans as needed
- Educate patients and caregivers on home exercise programs and injury prevention
- Collaborate with physicians, nurses, and other healthcare team members to ensure coordinated care
- Maintain accurate and timely clinical documentation in compliance with home health regulations
- Valid Florida Physical Therapist (PT) license in good standing
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
- Home health experience preferred but not required
- Current CPR/BLS certification
- Reliable transportation and valid driver's license for home visits throughout the Orlando area
- Strong clinical assessment, critical thinking, and communication skills
- Ability to work independently and manage a flexible PRN schedule
