Join our specialized team as a Full-Time Nurse Practitioner (NP) in Orlando, FL
. This crucial role focuses on providing comprehensive, hands‑on care to Medicare Advantage members residing in skilled nursing and assisted living facilities.
Compensation: This position offers a competitive pay rate ranging from $75 to $95 per hour
, depending on experience and employment type (Contract/Locums/Full‑Time).
Program Focus:
Treat‑in‑Place & Care Quality
Our program is dedicated to proactive disease management with a primary goal of treating in place when appropriate to avoid unnecessary hospitalizations and ensure a successful transition home. You will be instrumental in improving the overall quality of care for our members.
ResponsibilitiesRoutine & Acute Care: Provide primary, preventative, urgent, or acute care on‑site to facility residents, including hands‑on bedside care.
Comprehensive Assessments: Complete a thorough clinical assessment on each resident at a minimum of a monthly basis (more frequently as needed).
Care Planning & Orders: Develop a problem list, medical treatment plan, and create necessary orders for each resident in collaboration with facility staff.
High‑
Risk Management:
Stratify “high‑risk” patients for more frequent clinical visits and develop contingency plans for managing high‑risk conditions.Transition Care: Provide single visits (2–3 per day, approximately 90 mins each) to patients who have recently transitioned to a facility (SNF) as part of their hospital discharge plan, focusing on safe transition out of the facility.
PCP
Collaboration:
Communicate and collaborate directly with the resident’s Primary Care Physician (PCP) and facility staff (including the Director of Nursing‑DON) to manage the patient's overall care plan.Medication Management: Prescribe new or refill non‑narcotic medications upon consultation and agreement with the patient's PCP.
Goal Setting: Utilize advanced illness and disease trajectory training to establish personalized goals of care for residents.
Quality Metrics (STARS/HEDIS): Identify and address gaps in care for quality measures (STARS, HEDIS, COHPS, etc.), providing recommendations to the PCP for necessary adjustments.
Communication: Reach out to the responsible party/family after each visit to ensure clear lines of communication and care coordination.
Support Structure: An RN will assist the NP with Care Coordination tasks.
Panel Management: Manage a panel of members assigned to your care across multiple facilities (SNF, ALF, ILC, etc.). You will typically be scheduled at one facility per day but must be flexible to travel to another for an acute assessment if needed.
Employment Type: Full‑Time (Travel, Contract, Locums, Temporary options may also be available).
Work Days: Monday and Friday.
Hours: 8:00 AM – 5:00 PM.
Location: Greater Orlando, FL area.
Requires daily travel between local patient facilities.
Required Qualifications:
Education: Master of Science in Nursing (MSN) degree from an accredited school of nursing.
Licensure: Active and unrestricted Nurse Practitioner (NP) License in Florida
.Certification: Active ANCC or AANP certification in Family, Geriatrics, Adult, Adult‑Gerontology Primary Care, or Adult‑Gerontology Acute Care
.DEA & BLS: Current Drug Enforcement Administration (DEA) registration and current CPR certification (BLS).
A minimum of 1 year of current NP experience is preferred.
Prior experience in Skilled Nursing Facility (SNF) or Assisted Living (ALF) settings, Care Coordination, or Long‑Term Care (LTC) is highly valued.
