Are you a motivated LPN with home health experience who thrives in a fast-paced environment? Our growing home health agency is seeking a dynamic LPN Intake Coordinator to join our dedicated team in Maitland, FL

• Answer phones and manage fax communications
• Enter patient referrals into the EMR system
• Assist with coordination of home health services
• Create, modify, and maintain forms and records
• File, distribute Medicare paperwork, and track physician orders
• Assemble admission packets
• Support quality assurance and regulatory compliance efforts
• Jump in to help your team whenever needed

• Active LPN license (required)
• Valid driver's license & reliable transportation
• 1+ year of home health experience (required)
• Experience scheduling home health referrals (preferred)
• Proficiency with Microsoft Office Suite (Word, Excel, etc.)
• Ability to participate in on-call rotation
