PrimaryResponsibilities
- Works with providers and clinic staff to identify patients at high risk.
- Supports longitudinal care of patients with chronic conditions:
- Perform assessment of health conditions.
- Conduct medication reconciliation.
- Provide motivational interviewing and self‑management goal setting.
- Offer patient education and refer to appropriate agencies and resources.
- Supports transition of patients with chronic conditions from inpatient to outpatient settings:
- Assess transitional needs.
- Perform medication reconciliation.
- Establish and review contingency plans.
- Provide patient education.
- Assists with post‑discharge needs such as prescriptions, transportation, durable medical equipment (DME), and appointments.
- Coordinates with providers to establish or update the plan of care.
- Performs accurate and timely documentation in the electronic medical record.
- Participates in daily huddles and Patient Care Coordination (PCC) meetings.
- Prepares accurate reports for weekly meetings.
- Maintains continued competence in nursing practice and current evidence‑based practices.
- May perform clinical tasks within scope of practice.
- Bachelor’s degree in Nursing or Associate’s degree in Nursing with 4+ years of experience.
- Registered Nurse with an active, unrestricted license in the state of employment.
- Current BLS certification.
- 2+ years of experience in a physician’s office, clinic, or hospital setting.
- Proven knowledge of chronic diseases (COPD/asthma, diabetes, CHF, IHD).
- Proficient computer skills for electronic medical records.
- Proof of immunity to TB and required vaccinations:
Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis.
- Experience in patient education, motivational interviewing, and self‑management goal setting.
- Excellent verbal and written communication skills.
- Strong organizational and prioritization skills.
- Solid interpersonal and teamwork skills.
Pay ranges from $58,800 to $105,000 annually based on full‑time employment, education, experience, and certifications. In addition to salary, we offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and a 401(k) contribution.
Work Hours:
8 AM – 5 PM.
