RN Hospital Lead Case Manager

The Lead Case Manager supports the Care Management Operations and Senior Manager in overseeing Care Management staff, ensuring quality patient care, efficient resource use, and compliance with hospital guidelines. The Lead Case Manager is responsible for staffing, training, mentoring, and facilitating team workflows. The Lead Case Manager demonstrates critical thinking and creativity, supports change initiatives, and escalates issues to supervisors in the absence of managers.
Qualifications:
- Registered Nurse (RN) with acute hospital nursing experience required
- Bachelor’s Degree required
- Prior hospital experience in Care/Utilization Management in an Acute Hospital Setting required
- Bachelor's or Master’s in Nursing (BSN/MSN) preferred
- Certification in Case Management (CCM/ACM) preferred
Responsibilities:
- Staff Performance & Oversight:
Monitors staff performance, reports findings to the nurse manager, and takes corrective action as needed - Performance Improvement:
Demonstrates familiarity with the organization’s performance improvement model and supports unit activities through problem identification, assessment, data gathering, change implementation, and evaluation of outcomes - Process Improvement Initiatives:
Sponsors or participates in process improvement initiatives and ensures resulting practice changes are implemented - Quality Standards:
Supports departmental quality standards and initiatives - Critical Thinking & Leadership:
Applies critical thinking to planning, organizing, and leading the unit; incorporates data-driven and evidence-based decision-making - Technology Integration:
Demonstrates understanding of unit technology, integrates it into daily patient care, and leverages it to improve outcomes
