As the Home and Community Based Services Licensed Practical Nurse, you will be providing direct nursing care as prescribed by the physician to acutely and/or chronically ill patients in their homes, recording clinical and progress notes, and coordinating other patient care activities to deliver the highest quality of care in your assigned geographic service area. You will work under the direct supervision of the Registered Nurse and/or designated supervisor.
PrimaryResponsibilities
- Provides high‑quality clinical services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other members of the health‑care team, following all applicable infection‑control procedures.
- Documents patient visits thoroughly in the electronic medical record using the agency’s verification program, as required by regulatory and payer standards.
- Actively participates in the agency’s care‑coordination process, including timely reporting of patients’ needs and changes in condition, attending case conferences, and communicating effectively with the patient, family, physicians, clinicians, agency staff, and case coordinators.
- Current and unrestricted Licensed Practical Nurse license in the state of practice.
- Current CPR certification.
- Valid driver’s license, vehicle insurance, and access to a reliable vehicle or public transportation.
- Ability to function in any home situation regardless of the client’s age, race, creed, color, sex, disability, or financial condition.
- Home‑care experience.
- Ability to work flexible hours as required to meet client needs.
- Ability to manage multiple tasks simultaneously.
- Capability to work independently.
- Strong communication, writing, and organizational skills.
The hourly pay for this role ranges from $20.00 to $35.72 per hour based on full‑time employment.
