The Nurse Practitioner applies advanced education and clinical competencies to achieve optimal patient outcomes. The Nurse Practitioner works on problems of diverse scope and complexity ranging from moderate to substantial.

Driving is required;
Center Well provides transportation.

The Nurse Practitioner provides health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions; prescribes medication, examines patients, diagnoses illnesses, and provides treatment.

Advises executives to develop functional strategies (often segment‑specific) on matters of significance. Exercises independent judgment and decision‑making on complex issues regarding job duties and related tasks, working under minimal supervision and using analysis of variable factors to determine the best course of action.

• A Bachelor’s degree
• Eight (8) or more years of technical experience
• Two (2) or more years of project leadership experience
• Active, unrestricted Nurse Practitioner/PA license in the appropriate state
• This role is patient‑facing and part of Humana/Senior Bridge’s Tuberculosis (TB) screening program; TB screening is required if selected.
• Must be passionate about contributing to an organization focused on continuously improving consumer experiences
• Must be willing to participate in limited on‑call coverage by phone

The compensation range below reflects a good faith estimate of starting base pay for full‑time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job‑related skills, knowledge, experience, education, certifications, etc.

$115,200 - $158,400 per year

This job is eligible for a bonus incentive plan. The incentive opportunity is based upon company and/or individual performance.
