Triage Nurse (RN) Center Well West Orange

Center Well is seeking a hard‑working and dedicated Registered Nurse to promote and restore patients' health by completing the nursing process and who is interested in being part of a team that focuses on excellent service to others.

• Identifies patient care requirements by establishing personal rapport with patients. Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
• Responsible for evaluation of patients with ambulatory diagnoses as well as possible acute conditions. The triage nurse will work in partnership with the center's mid‑levels and PCPs.
• In consultation with the center's mid‑levels and/or PCPs the triage nurse will initiate treatment, if necessary, for common ambulatory diagnoses.
• Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self‑care skills; answering questions.
• Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.
• Resolves patient problems and needs by utilizing multidisciplinary team strategies.
• Work in other duties as assigned

• Must be willing and able to perform the following procedures or at least be willing to be trained to do them :
• 2 to 5 years of Urgent Care experience
• Starting an IV and initiating IV fluids
• Give Injections
• Simple wound care
• Insert a foley catheter if appropriate
• Administer aerosol treatments
• Perform EKGs
• Good communication skills
• Works well in a team of healthcare providers.

• Associates Degree or Bachelor of Science Degree in Nursing
• Nursing Diploma
• Bilingual in English and Spanish

Monday to Friday 8AM-5PM

Scheduled 40 hours per week

Local travel may be required;

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
