Job Description:- Other duties as assigned.
- Communicates with patients and families after discharge to address concerns and ensure understanding of follow-up care and functional assessment.
- Collaborates with the multidisciplinary team to ensure high-quality patient care.
- Participates in community education as needed. Maintains professional relationships with all customers and completes assigned work comprehensively and in a timely manner.
- Provides telephone and face-to-face consultations with patients, answering questions, providing educational information, and reviewing discharge information.
QUALIFICATION REQUIREMENTS:
Associate's of Nursing (Required), Bachelor's of Nursing Advanced Cardiac Life Support Cert (ACLS) - RQI Resuscitation Quality Improvement, Basic Life Support - CPR Cert (BLS) - RQI Resuscitation Quality Improvement, Registered Nurse (RN) - EV Accredited Issuing Body, Stroke Certified Registered Nurse (SCRN) - EV Accredited Issuing Body
Pay Range:$32.76 - $57.47
