• Conduct comprehensive assessments to gather data on patients' physical, emotional, and social health status.
• Develop and implement individualized care plans with specific goals and interventions based on assessment data.
• Administer medications and treatments accurately as prescribed, ensuring patient safety and adherence to protocols.
• Monitor and evaluate patient responses to interventions, adjusting care plans as necessary and documenting progress.
• Educate patients and families about health conditions, treatment plans, and self‑care strategies to empower them in managing their health.
• Facilitate effective hand‑off communication by clearly and concisely transferring patient information during shift changes or transitions between departments.
• Collaborate with healthcare professionals through regular communication and teamwork to ensure coordinated and optimal patient care.
• Maintain accurate and timely documentation of patient interactions, treatments, changes in condition, and care plans.
• Engage in continuous professional development by attending workshops, pursuing relevant certifications, and reflecting on practice to maintain clinical competencies and improve patient care.
• Oversee daily operations and patient care, ensuring compliance with policies, managing staffing needs, providing leadership, and addressing issues.
• Perform point of care testing to provide immediate diagnostic results, enabling timely decision‑making and treatment adjustments.
• Participate in performance improvement projects by identifying areas for enhancement, implementing changes, and evaluating results to improve patient care quality and outcomes.
