Invested Health Center is seeking an experienced Family Nurse Practitioner to join our mission-driven nonprofit primary care practice. Our providers deliver comprehensive primary care with a strong focus on preventive medicine, chronic disease management, HIV prevention and treatment, and sexual health in a collaborative outpatient setting. The Family Nurse Practitioner will provide comprehensive primary care while managing acute and chronic medical conditions, preventive care, and HIV prevention and treatment in a collaborative outpatient environment.

This position is ideal for an experienced provider who enjoys practicing high-quality medicine, building long-term patient relationships, and caring for a diverse patient population. Our providers have the opportunity to deliver thoughtful, evidence-based care while working closely with a collaborative clinical team in a growing nonprofit healthcare organization.

• Diagnose and manage acute and chronic medical conditions.
• Perform annual wellness visits and comprehensive new patient evaluations.
• Manage chronic diseases including diabetes, hypertension, hyperlipidemia, obesity, thyroid disorders, asthma, COPD, and more.
• Provide preventive care and health maintenance.
• Interpret laboratory and diagnostic testing.
• Coordinate specialty referrals and follow-up care.
• Educate patients on disease prevention, healthy lifestyle choices, and long-term wellness.

• Manage patients living with HIV.
• Prescribe and manage antiretroviral therapy.
• Initiate and manage PrEP, PEP, and DoxyPEP.
• Diagnose and treat sexually transmitted infections.
• Provide HIV prevention counseling and risk-reduction education.
