Pay Rate:$19.95

As a Case Manager at Aspire Health Partners, your job purpose to provide overall assessment and needs of chronically mental ill adults.

• Providing screening, referral, emergency, crisis and case management services,participating in clinical treatment team.
• Conducting intake assessments to determine family needs, efficiently documenting all activities,conducting activities with the client and families, providing regular input tothe treatment plan
• Attending community meetings to advocate for client needs, providing support and crisis intervention when necessary
• Documenting clinical records and additional program specific documents while maintaining the integrity of the client.
• Ability to use program technology to communicate effectively with staff
• On occasion, actas the authorized client transporter for necessary treatment and services.
• Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures.

• Bachelor's Degreein Social Work, Psychology or Human Services related field.
• Level II Background clearance
