Provide high‑quality physical therapy services in a home health setting, including assessments, treatment plans, therapeutic interventions, and patient education. Coordinate with support staff and multidisciplinary team members to manage caseloads, scheduling, and drive time efficiently.

• Graduate (current or pending) from an accredited physical therapy program
• Active or pending state physical therapist license
