Full time | Shift: Day (United States of America)

• Travels to patients' locations, including resident sites and hospital rooms, to assess eligibility for Hospice care.
• Partners with hospital Case Management staff to meet patient and hospital staff needs and gain new referrals.
• Assesses the emotional condition of patients and families to determine readiness for Hospice and address questions and concerns.
• Coordinates patient treatment efforts and promotes quality improvement and educational initiatives.
• Obtains required signatures and generates necessary electronic or paper documents for regulatory compliance and reimbursement.

Associate's degree in Nursing (Required), Bachelor's of Nursing, Basic Life Support – CPR Cert (BLS) – RQI Resuscitation Quality Improvement, Driver's License (DL) – EV Accredited Issuing Body, Registered Nurse (RN) – EV Accredited Issuing Body.
