• Provide direct nursing care to residents per individualized care plans
• Administer prescribed medications and document medication administration
• Monitor residents’ vital signs and clinical status; escalate changes to the supervising RN
• Assist with activities of daily living and resident mobility
• Maintain accurate charting and care documentation in the resident record
• Collaborate with the interdisciplinary team to ensure safety, quality of care, and regulatory compliance

• Active Licensed Practical Nurse license to practice in Maine
• Experience in long-term care or skilled nursing settings preferred
• Ability to perform 12-hour shifts in a travel assignment
• Strong communication, documentation, and teamwork skills
• Willingness to adhere to facility policies and infection control practices
