Key Responsibilities:

• Assess patient care needs and eligibility for services

• Link patients with appropriate service providers

Requirements:

• Membership in good standing with relevant regulatory body

• Recent community health experience

• Excellent communication and problem-solving skills

• Valid driver's license and reliable vehicle access
