Key Responsibilities:
• Conduct client assessments and planning personalized care
• Lead client care as the designated primary provider
• Facilitate connections to community support services
Requirements:
• Foot Care Certification is mandatory
• Must travel within Durham Region
• Experience with diabetes care
• Certificate of Competence from Ontario’s nursing college
• Valid driver’s license and access to a vehicle
