Shape the future of elder care at Durham Community Health Centre as a Nurse Practitioner. This temporary, full-time role, based in Oshawa, emphasizes community-based geriatric services.
As a member of the GAIN Community team, you will provide essential support to elderly clients, focusing on conducting comprehensive assessments and delivering personalized care. This role also includes collaboration on integrative care plans and outreach services, ensuring that clients receive holistic support tailored to their needs. Engage in both clinical practice and administrative duties.
Key Responsibilities:
• Assess and manage complex geriatric health issues
• Develop comprehensive treatment plans for seniors
• Collaborate with a multidisciplinary team for care
• Facilitate client outreach assessments within the community
• Maintain robust documentation and reporting standards
Requirements:
• Master's Degree in Nursing with NP qualifications
• Three years’ experience with elderly populations
• Advanced skills in wellness assessment and coordination
• Demonstrated leadership in clinical practice
• Valid Ontario Driver's license with vehicle accessibility
