Excel in a temporary role as an Interdisciplinary RAI Coordinator at Age Care, specializing in MDS processes and comprehensive team education. Improve residents' assessment experience within long-term care.
This temporary position focuses on the administration and coordination of MDS documentation by leading the interdisciplinary team in compliance with healthcare standards. You will provide essential training and support while monitoring timeliness and accuracy in MDS assessments. Key Responsibilities:
• Lead the organization and education of MDS documentation
• Monitor and provide support to interdisciplinary team users
• Develop a clear schedule for resident assessments
• Audit MDS completion and coding accuracy
• Facilitate MDS training using a train-the-trainer approach
Requirements:
• Valid Registered Nurse or Registered Practical Nurse license in Ontario
• At least three years of experience in Long Term Care settings
• Deep understanding of Long Term Care Act and MDS standards
• Proven leadership and project coordination skills
• Commitment to resident-centered care and support
