Position: Youth Advocate Outreach Worker Role
Work with Durham Community Health Centre as a Youth Outreach Worker and make a difference in the lives of at-risk youth in Oshawa and Pickering. This role centers on health advocacy and community engagement.

The Youth Outreach Worker will connect with vulnerable youth aged 12-21, offering guidance and support as they navigate complex social services. This full-time position includes the development and delivery of educational and harm reduction programs, alongside strong teamwork to foster health equity across the community. Your collaboration will drive meaningful change in youth wellness.

Key Responsibilities:

• Conduct outreach to connect with at-risk youth

• Provide necessary referrals and support services

• Develop and implement health promotion initiatives

• Assist in maintaining client documentation

• Build relationships with community service partners

Requirements:

• 2-year diploma in Health or Social Services

• At least one year of relevant community experience

• Familiarity with vulnerable populations and health challenges

• Strong advocacy and negotiation skills

• Valid driver’s license with travel readiness in Durham
