Join Ontario Shores as a dedicated Social Worker focusing on care coordination and community liaison. This role is crucial in linking patients to vital services for effective mental health outcomes.

In your capacity as a Social Worker, you will employ evidence-based models to address patient needs and engage with community resources. Your tasks will include monitoring referrals, facilitating access to treatments, conducting assessments, and collaborating with your interprofessional team. Additionally, you will engage in community education and support student learning.

Key Responsibilities:

• Provide recovery-focused treatment and rehabilitation

• Coordinate effective discharge plans and psychosocial assessments

• Advocate on behalf of patients to connect them with needed services

• Maintain collaborative partnerships with community organizations

• Ensure records adhere to OCSWSSW standards

Requirements:

• Bachelor’s of Social Work degree mandatory

• Registered Social Worker in Ontario

• Valid Ontario Driver’s License

• Knowledge of recovery-based social work principles

• Excellent communication and organizational skills
