Mental Health Case Manager Position

As a Case Manager, your focus will be on implementing supportive care plans that address social determinants of health while ensuring client understanding of services. Collaborate within a multi-service team to adapt treatment goals as needed. You’ll help clients cultivate life skills necessary for independent living and community stability.
Key Responsibilities:
• Process client intakes and develop care plans
• Support clients with medical appointments and transportation
• Maintain documentation and prepare detailed reports
• Empower clients through life skills teaching
• Deliver medication management and monitoring
Requirements:
• Post-secondary education in human services or related field
• At least one year in a similar role
• Familiarity with applicable Ontario legislation
• Ability to build therapeutic relationships
• Flexible schedule with availability for evenings
