Enhance your career as a Case Manager with Canadian Mental Health Association Durham. This role involves providing vital support to clients facing mental health and addiction challenges through a hybrid working model.
As a Case Manager, you will advocate for and support clients within the HART Hub team. Your responsibilities include developing individualized care plans and collaborating with multi-disciplinary professionals. You'll contribute to client recovery and promote independence by focusing on life skills and community connections.
Key Responsibilities:
• Collaborate with multidisciplinary teams in client support
• Process client intakes and develop supportive care plans
• Teach life skills for healthier community living
• Document client contacts and create timely reports
• Ensure safety and continuity of care for clients
Requirements:
• Post-secondary education in mental health or related field
• Minimum one year of relevant experience
• Valid Ontario Driver’s license with a dependable vehicle
• Knowledge of community resources and legislation
• Strong communication skills and ability to work independently
