$30.15/hr - $45.25/hr

Major Responsibilities
  • Daily, provides ongoing Medical Nutrition Therapy (MNT) care for the entire hospital inpatient census, through screening, prioritizing, assessing, and recommending interventions. Adapts daily workflow to fluctuations in patient load due to census variations, consults, acuity, and staffing. Develops and implements a plan of care for patients based on assessment of life cycle- and diagnosis-specific nutritional needs. Assessment includes reviews of medical history and laboratory data, patient interviews, and physical examinations to determine current nutritional status.

    Documents MNT plans in the electronic medical record and supports coding standards to maximize reimbursement.
  • Prescribes enteral and parenteral nutrition support orders by assessing the patient and calculating macronutrients and/or micronutrients, fluids, and electrolytes. Evaluates tolerance and effectiveness of formulas and manages modifications to enteral/parenteral regimens. Demonstrates knowledge of specialized nutrition support and collaborates with physicians and pharmacy. Maintains secure communication with the Pharmacy Integrated Clinical Services (PICS) team to manage Total Parenteral Nutrition (TPN) infusions for critically ill patients.
  • Prescribes therapeutic diets, oral nutrition supplements, vitamins/minerals, fluid management, and related lab work within the Medical Nutrition Therapy (MNT) protocol. Writes orders to implement nutrition-related plans and collaborates with Food and Nutrition services to align intake with the nutrition prescription and care plan. Partners with site teams to identify substitutions during supply disruptions to minimize impact on patient care. Ensures diet restrictions and preferences align with patient needs and cultural considerations.

    Develops patient-specific menus for complex dietary needs.
  • Leads efforts to identify, treat, and prevent malnutrition. Performs Nutrition Focused Physical Examinations for high-risk patients to evaluate malnutrition and communicates findings to the medical team to maximize care outcomes. Provides nutrition education to patients with malnutrition to improve outcomes and reduce readmission rates.
  • Documents elements of the Global Malnutrition Composite Score (GMCS) for CMS nutrition quality indicators. Participates in Quality Assessment Performance Improvement (QAPI) to ensure safe and effective nutrition care. Maintains regulatory readiness and supports department initiatives for safety, policies, and procedures, including regulatory standards.
  • Identifies and documents billable nutrition diagnoses with supporting documentation. May support outpatient nutrition programs and enters charges for outpatient nutrition services in the EMR.
  • Partners with local didactic programs by precepting dietetic graduate students, interns, and residents. Provides nutrition lectures and orientation as applicable and communicates with program coordinators.
  • Collaborates with discharge planners, home health, physicians, pharmacy, and other teams to coordinate nutrition goals. Participates in rounds and care conferences and assists with transitions to home or other facilities as needed.
  • Provides nutrition education to patients on prescribed diets and educates family members as needed. May provide in-services to teammates.
  • Provides clinical input into system nutrition initiatives and practice changes. Participates in system projects to deploy changes to menus, formulary, charting systems, safety procedures, and standards. Acts as a liaison between clinical nutrition and inpatient teams to implement nutrition initiatives across sites as needed.
Licensure, Registration, And/or Certification Required
  • Dietitian certification issued by the Commission on Dietetic Registration (CDR)
  • Dietitian (RD) registration with the Academy of Nutrition and Dietetics
