Caregiver in-home

Community Horizons is seeking a dedicated Caregiver in-home to support a client in the Oshkosh, WI area.

Responsibilities
  • Assist with daily living activities, including personal hygiene and independent life skills.
  • Prepare and serve meals tailored to the client’s dietary requirements.
  • Act as a coach and cheerleader, helping the client set and achieve personal goals and promote independence.
  • Engage the client in enjoyable recreational activities and outings to maintain social connections and activity.
  • Maintain a safe, clean home environment through light housekeeping duties.
  • Ensure safety and well‑being by adhering to policies and documenting care, including safety checks and care logs.
  • Pick‑up clients on weekends when required to meet a minimum of 40 hours per week.
Qualifications
  • 3‑5 years of relevant caregiving experience.
  • Successful completion of a caregiver background check.
  • Valid driver’s license, auto insurance, and a clean driving record.
  • Strong interpersonal skills and the ability to work independently as part of a team.
  • Commitment to “family‑oriented culture” and flexible scheduling.
Work Schedule & Pay
  • Full‑time (40 hours per week).
  • Work schedule:

    Monday–Friday, 4 pm–10 pm (excluding Thursdays); 8 am–4 pm with optional weekend pick‑ups.
  • Competitive hourly rate of $20/hour.

About Community Horizons

Community Horizons provides support to individuals and families enrolled in self‑directed care programs across Wisconsin, including Managed Care Organizations, IRIS (Include, Respect, I‑Self‑Direct), and the Children’s Long‑Term Support (CLTS) program. Participation in self‑direction allows clients to choose their caregivers and how care is delivered. Community Horizons offers guidance and mentorship but is not the employer of record; a third‑party fiscal agent handles payroll and employment paperwork.
