FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one care to older adults in Appleton, WI, in private homes or senior living communities. You will conduct evaluations, develop plans of care, and provide evidence-based treatment within the Geriatric House Calls™ model.
You’ll build a flexible schedule, may use telehealth, collaborate with PTs, OTs, SLPs, and Fitness Specialists, and educate patients and caregivers to promote safety and independence.
