Purpose and

Summary:

Ensures residents receive necessary care and services through a multidisciplinary approach to attain or maintain their highest practicable physical, mental, and psychological well‑being. Provides nursing care and assessment for establishing interventions within a plan of care and an environment that promotes maintenance or enhancement of quality of life, following Park View Health Center policies, Federal and State regulations, the Nurse Practice Act, and nursing assessment process.

Functions
  • Delivers skilled nursing care, administers medications as ordered, transcribes physician orders, and ensures direct care is provided skillfully, safely, completely, and accurately.
  • Manages, directs, and provides assignments to certified nursing assistants, guiding delegation of direct nursing care tasks, assigning neighborhood tasks, monitoring workflow, communicating with the CNAs team, and providing follow‑up guidance and education.
  • Completes accurate nursing assessments in a timely manner, identifies resident changes in condition, directly assesses and observes residents with changes, documents observations and assessments, follows up with appropriate changes to nursing short‑term and comprehensive care plans, and completes follow‑up assessments for identified resident and neighborhood concerns.
  • Additional Essential Duties
  • Accurately and timely documents resident assessments, pertinent information, weekly progress notes, notifications to POAHC/guardians, and notifications of the resident’s physician/physician extender.
  • Critically thinks through resident concerns, forms problem‑solving solutions, consults with interdisciplinary team members, discusses concerns directly with residents and families, and offers assessment information to supervisors.
  • Maintains effective and professional communication with physicians, physician extenders, community healthcare entities, all interdisciplinary departments, supervisors, and administration.
  • Understands nursing department chain of command and reports acute incidences or concerns timely to RN shift supervisor and RN neighborhood supervisor.
  • Demonstrates ability to organize and prioritize daily assignments, completing medication pass, resident assessments, documentation, and physician order transcription within scheduled shift.
  • Accurately transcribes newly signed physician orders, visits, and consultant visits into resident's medical record.
  • Shows flexibility in work hours and assignments, adapting to changing environment and interruptions; staff nurses work every‑other‑weekend and holiday rotation.
  • Monitors neighborhood supplies for resident care, ensuring appropriate quantities are maintained (medications, skin or wound care supplies, tube feeding or oral nutritional supplements, Foley catheters, and adaptive equipment).
  • Completes consents for medications and safety equipment, delivering education and obtaining signatures.
  • Thoroughly assesses resident vital signs, labs, diagnostic tests, and consultation reports for acute concerns, communicating concerns to primary physicians.
  • Participates in admission, transfer, and discharge processes for residents.
  • Functions as a resident advocate, responding promptly to requests and concerns.
  • Assists certified nursing assistants with resident direct care including transfer assistance, meal‑time assistance, and obtaining vital signs.
  • Supports guidance and education in policy and procedure to prevent and prohibit mistreatment, neglect, and abuse of residents.
  • Demonstrates a professional demeanor, maintaining a positive, cooperative attitude, poise during stressful situations, courteous communication with residents, visitors, employees, interdisciplinary team members, supervisors, and administration, and accepting guidance from nursing management.
  • Performs other related duties as assigned.

    Experience in geriatric nursing field is desirable.
