Job SummaryThis role operates administratively in a Home and Community-Based Services (HCBS) setting, providing medical, social, and supportive services for individuals with functional, cognitive, and other physical or mental health needs. The programmer supports teams in monitoring services, creating strategies to reach goals, and coordinating services to empower people to meet and exceed their goals.
Responsibilities- Create and maintain team culture through active presence, monthly meetings, coaching, and on‑call availability.
- Oversee resources including vehicle usage, budget monitoring, representative payee coordination, ledgers, receipts, and medication check‑ins.
- Ensure services honor individuals’ needs, desires, and goals by serving as primary point of contact with Medicaid CBCM, inputting accurate EMR data, developing individualized plans, updating authorizations, and conducting quarterly reviews.
- Communicate timely with individuals, families/guardians, and case managers regarding service changes and document interactions.
- Collaborate with the Interdisciplinary Team to write support goals and personal outcome plans within 21 days of new individuals.
- Promote community partnerships and local services with the Strategic Advancement team.
- Comply with all policies, laws, and governing rules and provide accurate documentation of incident reports and investigations.

- At least 18 years of age.
- Valid driver’s license and reliable transportation with required liability coverage.
- High school diploma or GED equivalent.
- U.S. work authorization without sponsorship.
- Competitive wages: $22.63 per hour (potentially higher with experience).
- Full‑time schedule Monday‑Friday, 8:00 a.m. to 4:30 p.m., with on‑call rotation and coverage flexibility.
