Je voert brede gesprekken, beoordeelt indicaties en maatwerkvoorzieningen, werkt met wijkteams en Zorgpartners en registreert besluiten in Zorg Ned.
Je hebt minimaal HBO werk- en denkniveau, kennis van Wmo, Wlz en Zorgverzekeringswet, en ervaring met Wmo-aanvragen. Er is ruimte voor ontwikkeling en een marktconforme beloning.
